Health Services Clinic
Where: Memorial 118 on CIU main campus
Hours: Mon-Fri 9:00 a.m.- 4:00 p.m. (closed for chapel, lunch, holidays & prayer days)

Services are provided for CIU students only and consist of the following:
* Walk-in clinics, Monday - Friday, for nursing assessment and recommendations
* Doctor's clinic by appointment weekly during fall and spring semesters (Minimal fee)
* Referrals (medical, dental, chiropractic, etc.)
* Many over the counter medications at low cost
* Screening programs

Vaccine/Health Requirements Policy for Course Registration
For students taking courses on the CIU Columbia campus during a regular semester, the following requirements apply:
1. Students born after 1956 must provide proof of immunity to measles and rubella with either official immunization records or proof of immune blood titer tests
2. All students must provide proof of a Tuberculosis (TB) skin screening test done within the year prior to enrollment
 3. All students must provide proof of insurance coverage or purchase a policy available through CIU
Health forms are due by August 1 for August registration and by December 15 for Spring registration.     

Health Requirements

Please note: Besides insurance information and immunization records, the form requires a TB (Tuberculosis) Test current within the year. Make sure to have the test results listed on the form and have the completed form in by your Orientation Session.
